Forms

		Forms needed to enroll an infant: 1,2,3,4,6,8,9,10, and 11.
		Forms needed to enroll a child 2-5 years old: 1,2,3,4,6,10, and 11.
		Forms needed to enroll a child 5 years and older: 1,2,3,4,7,10, and 11.

       (816) 420-0800. You will need to provide us with the ages of the
       children and a valid fax number.

		1. Child Enrollment Form- This form must be completed
		prior to attendance and needs to be completed again anytime there is a change in personal information.

		2. Contract- This form must be completed prior to
		attendance.

		3. Authorization For Contact and Pick Up- This form must
		be completed prior to attendance.

4. Personal Information-This form must be filled out.

		5. Tuition Express- This form is needed to pay tuition
		electronically.

		6. Child Medical Examination Report
		(infant/toddler/preschool)- This form must be completed by

a physician within thirty days of enrollment.

		7. Parent's Health Statement for School Age Child - Parents
		must complete this form within thirty days of enrollment.

		8. Parent's Specialized Instructions for Infants and Toddlers -
		Parent's of Infants and Toddlers must complete this form prior to their children's attendance.

		9. Infant Feeding Preference - Parent's of Infants and Toddlers
		must complete this form prior to their children's attendance.

		10. Income Eligibility Form CA CFP - This form must be
		completed prior to attendance and needs to be redone ever calendar year.

		11. Enrollment Forms (schedule) CACFP- This form must
		be completed prior to attendance and needs to be update ever calendar year for three years, on the fourth year a new form must be completed.
